Why vets run blood work
Blood tests give your vet a snapshot of what's happening inside your dog's body โ things that can't be seen during a physical exam. They're routine during annual wellness visits, pre-surgery screening, and when your dog is sick.
Most vet labs run two main panels: a complete blood count (CBC) and a chemistry panel. Some add a thyroid test. Together, these cover organ function, infection signs, hydration, and more.
The printout you get back is usually a wall of abbreviations and numbers. Here's what the major ones mean in plain language.
The complete blood count (CBC)
A CBC measures the cells in your dog's blood โ red cells, white cells, and platelets. It helps your vet spot infections, anemia, clotting issues, and inflammation.
Red blood cells (RBC) and hematocrit (HCT)
Red blood cells carry oxygen through the body. Hematocrit is the percentage of blood volume made up of red blood cells โ think of it as how "thick" or "thin" the blood is with oxygen carriers.
A low hematocrit can indicate anemia, which has many causes โ from blood loss to chronic disease. A high hematocrit sometimes shows up with dehydration (the blood is more concentrated because there's less fluid).
A good question for your vet: "My dog's hematocrit is [high/low] โ is this something we should recheck, or does the rest of the panel explain it?"
White blood cells (WBC)
White blood cells fight infection and respond to inflammation. The CBC breaks them into subtypes:
- Neutrophils โ the first responders. High counts often point to bacterial infection or stress. Low counts can mean the bone marrow is overwhelmed.
- Lymphocytes โ part of the immune system's longer-term response. Can be elevated with certain infections or, rarely, lymphoma.
- Eosinophils โ often elevated with allergies or parasites.
- Monocytes โ can increase with chronic inflammation or recovery from infection.
A good question for your vet: "Which white cell type is off, and what does that narrow it down to?"
Platelets (PLT)
Platelets help blood clot. Low platelets (thrombocytopenia) can mean your dog bruises or bleeds more easily. Causes range from tick-borne diseases to immune conditions. Very high platelet counts sometimes appear with inflammation or iron deficiency.
The chemistry panel
The chemistry panel measures substances in the blood that reflect how organs are working โ especially the kidneys, liver, and pancreas.
Kidney values: BUN and creatinine
BUN (blood urea nitrogen) and creatinine are waste products filtered by the kidneys. When both are elevated, it often means the kidneys aren't filtering as efficiently. But BUN alone can rise from dehydration or a high-protein meal, so context matters.
Some labs now include SDMA, a newer marker that can catch kidney changes earlier than BUN or creatinine alone.
A good question for your vet: "Are the kidney values elevated enough to warrant a follow-up urine test?"
Liver values: ALT, ALP, and bilirubin
ALT (alanine aminotransferase) rises when liver cells are damaged or inflamed. ALP (alkaline phosphatase) is trickier โ it can come from the liver, bones, or intestines, and it's commonly elevated in young growing dogs or dogs on steroids. Bilirubin is a byproduct of red blood cell breakdown, processed by the liver.
A mildly elevated ALP in an otherwise healthy dog on no medications often means nothing worrisome. A significantly elevated ALT with symptoms is a different conversation.
A good question for your vet: "Is this liver value high enough to need an ultrasound or more specific liver tests?"
Blood sugar: glucose
High glucose can indicate diabetes, but it also spikes from stress (especially in anxious dogs at the vet). Persistent high glucose across multiple tests is more meaningful than a single reading. Low glucose is less common but can be serious in small breeds or puppies.
Pancreas values: lipase and amylase
These enzymes are produced by the pancreas. Elevations can point to pancreatitis โ inflammation of the pancreas, which can cause vomiting, abdominal pain, and loss of appetite. Your vet may run a more specific test called cPLI (canine pancreatic lipase immunoreactivity) to confirm.
Electrolytes: sodium, potassium, chloride
Electrolyte imbalances can signal dehydration, kidney issues, Addison's disease (low sodium + high potassium is a classic combination), or digestive problems. They're rarely interpreted in isolation โ your vet looks at the pattern across all values.
Protein: total protein and albumin
Low albumin can mean protein is being lost through the gut or kidneys, or that the liver isn't producing enough. High total protein sometimes appears with chronic inflammation or dehydration.
Thyroid panel
Hypothyroidism (low thyroid function) is common in dogs, especially middle-aged and older dogs. The main test is T4 (total thyroxine). A low T4 with symptoms like weight gain, lethargy, or skin changes often leads to a more detailed panel (free T4, TSH) to confirm.
Thyroid medication is usually straightforward once diagnosed, but the initial interpretation isn't always clear-cut โ some medications and illnesses can temporarily lower T4 without true hypothyroidism.
A good question for your vet: "Is the low T4 a true thyroid problem, or could it be affected by something else going on?"
What "flagged" values actually mean
Lab reports mark values outside the reference range with flags โ an H for high, an L for low, or sometimes a color-coded highlight. This is the part that causes the most anxiety for pet owners.
Here's what most vets would tell you if they had more time:
- One slightly off value surrounded by normals is often meaningless. Bodies aren't perfectly calibrated machines. A value 5% outside the reference range is rarely a crisis.
- Trends matter more than snapshots. A creatinine of 2.0 in a dog whose creatinine has always been 0.9 is very different from a dog whose creatinine has always been 1.8. This is why keeping historical records is so valuable.
- Clusters of related values tell a story. Elevated BUN + creatinine + phosphorus together is a kidney pattern. One of those alone could be noise.
- Context changes everything. A stressed, dehydrated dog at the vet will have different values than the same dog calm and hydrated at home.
How to get more from your vet visit
Blood work conversations are often rushed โ your vet has 15 minutes and a lot to cover. Here's how to make the most of it:
- Bring previous results. Trends across tests are far more informative than a single snapshot. If you've used different vets or clinics, consolidate the records so your current vet can compare.
- Ask about the pattern, not individual numbers. "What does the overall picture look like?" is a better question than "Why is this one number high?"
- Ask what to watch for at home. If values need monitoring, your vet can tell you what behavioral or appetite changes would warrant a recheck sooner.
- Request a copy. You're entitled to your pet's lab results. Having them on hand helps at emergency visits, specialist appointments, or if you switch vets.
